What is France's 'franchise medicale'?
"You might hear about the franchise médicale from time to time when the French government weighs increasing it to finance the social security system. The franchise médicale is applied to prescriptions, visits to 'paramedical' professionals (including appointments with nurses, physiotherapists and masseurs, speech therapists and eye specialists), and non-emergency medical transport. It is not applied to any medicine given out during a hospitalisation or emergency transport, neither does it apply to normal doctor appointments."
"How much is the franchise médicale? As of October 2025, the franchise médicale was set to €1 per box/package of medication, €1 per paramedical procedure, and €4 for non-emergency medical transport. The franchise médicale has a daily cap for paramedical services and non-emergency medical transportation. You cannot be charged more than €4 per day for the former, and you cannot be charged more than €8 per day for the latter. Overall, you cannot be charged more than €50 per year for the franchise médicale."
The French system reimburses medical costs after payment, with most remaining costs covered by a mutuelle. A small patient contribution called the franchise médicale applies to prescriptions, paramedical visits and non-emergency medical transport, but not to medicines given during hospitalisation, emergency transport, or normal doctor appointments. As of October 2025 the charges were €1 per medication box, €1 per paramedical procedure and €4 for non-emergency transport, with daily caps (€4 for paramedical services, €8 for transport) and an annual cap of €50. Exemptions include children under 18 and beneficiaries of complementary or state medical aid.
