An SFPD internal investigation concluded that rookie officer Chris Samayoa acted within department use-of-force policies when he shot and killed 42-year-old Keita O'Neil through a squad car window during a 2017 pursuit. Samayoa and his training officer chased an unarmed O'Neil as he purportedly fled in a white lottery van. Investigators found violations only for body-worn camera and seat belt policies; Samayoa was not wearing a seatbelt and activated his camera only after the pursuit ended. Former Chief Bill Scott cited tactical concerns, premature gun deployment, lack of experience, and inadequate guidance from the field training officer. O'Neil's mother received a $2.5 million settlement and Scott later dismissed Samayoa and opposed his reinstatement.
