Utah State University showed patterns of financial noncompliance among leaders and staff and experienced poor oversight by the Utah Board of Higher Education. Twenty-six recommendations for improvement were issued. Renovations to the former president's office proceeded without secured funding, escalating from an intended $10,000 to $300,000 and remain unfunded. The university contracted vendors with professional ties to leadership and sometimes procured work without competitive processes. Personnel used costly accommodations during work travel. Leadership violations went unpunished, undermining internal controls. A new president, Brad Mortensen, was hired and new rules for reviewing presidential spending were instituted.
